New Hire Payroll Information Form is used for Payroll Partner clients to provide new employee information, or change information on existing employees. I-9 Form is required for all employees, and verifies the employee’s status as legal for employment in the US. Employee Direct Deposit Enrollment Form allows employees to specify financial information required for direct deposit of their pay. W-4 Form allows employees to specify their withholding information. FLSA With Plans OMB No. 1210-0149 is for employers who offer a health plan to some or all employees. FLSA Without Plans OMB No. 1210-0149 is for employers who do not offer a health plan.
